For a monthly fee of $25 or $250 per year (plus applicable taxes) an individual could make 2 appointments per month with any practitioner at any of our clinics. These would include initial consultations, or follow up, but exclude consumables (acupuncture needles, lotions used in massages, etc.). All consumables would be billed separately, and at cost. Any months where the limit of 2 was not filled, would not carry over to the following month. Additional visits would be available at a 25% discount off the published billing rates of the practitioner. Products would also be available at subsidized costs to members of the plan.
Level
TWO
Target market: Individuals that have begun adopting
some preventative care.
Level THREEFor a monthly fee of $49 or $475 per year (plus applicable taxes) an individual could make 5 appointments per month with any practitioner at any of our clinics. These would include initial consultations, or follow up, including consumables (acupuncture needles, lotions used in massages, etc.). Any months where the limit was not filled, would not carry over to the following month. Additional visits would be available at a 35% discount off the published billing rates of the practitioner. Products would also be available at subsidized costs to members of the plan.
For a monthly fee of $99 or $999 per year (plus applicable taxes) an individual could make 6 appointments per month with any practitioner at any of our clinics. If the practitioner recommends more visits, they would be allowed up to a maximum of 9 per month. These would include initial consultations, or follow up, including consumables (acupuncture needles, lotions used in massages, etc.). Any months where the limit was not filled, would not carry over to the following month. Additional visits would be available at a 50% discount off the published billing rates of the practitioner. Products would be included for members of the plan providing the practitioner makes a written recommendation. Certain limits apply. The products would be for new conditions or issues and for curative purposes, not performance enhancing.

FOUR
Target market: Individuals that have made alternative
health care their only source of wellness and health care
For a monthly fee of $175 or $1,999 per year (plus applicable taxes) an individual could make unlimited appointments per month with any practitioner at any of our clinics (subject to availability). These would include initial consultations, or follow up, including consumables (acupuncture needles, lotions used in massages, etc.). Products would be included for members of the plan providing the practitioner makes a written recommendation. Certain limits apply. The products would be for any conditions or issues and for curative purposes, as long as they were disclosed at time of application. Performance enhancing products would be provided, with limits, as long as recommended by practitioner.
Group
and family discounts apply and are arranged on a case-by-case basis.
Custom plans can also be arranged for critical care patients or acute
conditions.
